| Details: | Attempted to contact Faith Bible West in response to violations. The school has been sold and old operator was not aware of new contact information. A site visit was made to try and find out who is the new operator. A preschool teacher was onsite and they were not aware of any owner/pastor/water system operator contact information. I knocked on the residence of the pastor and no one was home. A letter has been sent to the preschool address requesting sampling compliance and an update on the contact information. |
