| Details: | 4/22/24 to 4/25/24 called 3 times to attempt to schedule the drinking water survey and spoke to Shada Tenney at least once, who advised she would have her dad (Brian Tenney) return his call to schedule (Peter Cooley). 4/26/24 visited the West Coast Game Park to deliver two coliform sample bottles, paperwork, and the "Opportunity" letter, and to try to persuade the operator to collect water samples prior to the end of April, and another sample for the present quarter to stave off the coming monthly coliform sampling requirement due to missing several quarters of coliform sampling. Shada Tenney (owner’s daughter) was not comfortable allowing us to perform a system survey, and called Brian Tenney, who said he would get back to Peter the following week. We reiterated that timing was critical with the sampling if monthly coliform sampling was to be avoided (Peter Cooley). 8/5/24 called WCGP and spoke to Shada Tenney, informed her that Rick would stop on 8/7 to see if owner Brian Tenney would allow access for water system survey at that time. Shada said that Brian Tenney would be notified of intent. (Donna Nelms) 8/7/24 stopped at WCGP about 9:45 am for purposes of completing a water system survey. With the business door locked attempted contact by phone call with no response. Shortly before 10 am an employee asked my purpose in waiting. I asked to see Brian Tenney and she left for a few minutes returning to say he was not on the property. I provided my contact info with a request to invite Brian to call me to schedule a survey (Rick Hallmark). 8/8/24 Letter was mailed to WCGP/Brian Tenney stating in part: Every few years someone from the local public health department has the assignment to visit West Coast Game Park to survey the water system. I would like your permission to do the survey and preferably at a time you can be present.
