Details: | Level 2 investigation was conducted due to having a second level 1 trigger within a 12 month rolling period. The investigation found that the Mink booster sample point was not a good sampling station due to being low to the floor and close to the wall. Operator thinks there was some splashing which could have caused the two +TC detections at this sample station. Cooler for transporting the sample was dirty. Crew did not have materials for proper sanitizing of their hands. Bleach mixture for sanitizing was very weak. Also advised samplers to not rush and follow complete sampling protocols. Corrective actions: The operator raised sampling port and angled it at 45 degrees to lessen the chance for splash back. Operator reviewed proper sampling protocols with sampling staff and advised to heat sample tap with torch in case of potential contamination. Raised and angled away from wall. Built a kit, spray bottle with bleach mixture, torch, nitrile gloves and Clorox wipes, for sampling. Washed and sanitized transport cooler. Advised crew to keep fresh and stronger bleach mixture. Updated sampling S.O.P. to help with proper sampling. Will enforce the cleanliness of our sampling routine to a higher degree and discuss procedures and stress the importance of the sampling. Another investigative coliform sample is being taken at this sampling site to see if there are issues within the booster station - result was negative for coliforms. Operator drained pressure tanks, refilled, and flushed twice again. |
