| Details: | 9/27/24 - DWS contacted system regarding routine TC+ sample reported on 9/26/24. Hayden claims to have rushed through sample collection. As a result, the sample location was not properly disinfected or flushed prior to sample collection. Instructed the system to collect three repeat samples and submit them to the lab within 24 hrs of the TC+ report. 9/30/24 - Hayden confirmed that all repeat samples were TC negative. |
