| Details: | Cindy requested more time to correct outstanding survey deficiencies. The operator agreed that they should be able to correct all remaining survey deficiencies within an additional four months. Given that this system was recently re-classified to a community-system, and that it has been making ongoing progress on outstanding deficiencies, I extended the due dates out an additional four months for the remaining outstanding deficiencies. Cindy stated that they are considering contacting a 3rd-party to act as the certified operator for their system, that they will request additional circuit-rider assistance to complete a cross-connection plan, that they are working on the operations & maintenance manual, and that they are working on the emergency response plan. |
