| Details: | Called Mary, and left a message regarding the outstanding deficiencies from the survey conducted on 10/12/2018. Awaiting call back to establish a corrective action plan to resolve their deficiencies. 2/20/2019 E-mailed Marc McKay to inform him of the outstanding significant deficiencies. Once we establish if any progress has been made on these items or not, we can set up a corrective action plan to resolve them. 3/8/2019 Called Mary to get a phone number for Marc after I received no response from my email to him. Called 503-369-7684 (Alleged Operator) and left a message regarding outstanding significant deficiencies. 3/13/2019 Called 503-369-7684 again. Spoke with alleged operator's partner. I was told he no longer works as the system operator. At this time, the water system has several outstanding significant deficiencies as well as no operator. Sent a letter regarding Water System Survey Significant Deficiencies/Rule Violations to Mary Le on 3/13/2019. |
