| Details: | I contacted Grant via email and sent him the following information: Hi Grant, Olds Mobile Park now has a violation for not submitting a lead service line inventory. The violation that can be viewed online at: https://yourwater.oregon.gov/violsum.php?pwsno=00392 . Failure to submit an initial LCRR service line inventory is a 5 point violation and requires the system to issue a public notice (see attached template). Please send me a copy of your completed notice once it has been issued. For information on submitting the lead service line inventory visit: https://www.oregon.gov/oha/PH/HEALTHYENVIRONMENTS/DRINKINGWATER/RULES/Pages/LCRR.aspx There is an excel template that needs to be completed and submitted to DWP.DMCE@odhsoha.oregon.gov .
