| Details: | I stopped by the facility, which is a licensed restaurant, to request a water survey. I had previously called/left a voicemail for the operator on 4/3/2025 but got no reply. The facility was closed, so I left a written survey request and instructions at the front door. I spoke with the operator's partner/husband and requested they contact me to schedule a water survey. I also sent emails and voicemails the following week requesting a time/date for the survey. The operator left me a voicemail the week of 4/14 suggesting May 1st, but did not select a time for the survey as requested. I again called/emailed the operator requesting that they pick a time to do the survey on 4/25/2025. As of 4/30/2025 the operator still has not responded or picked a time to do the water survey. | ||
