| Details: | Ken Hicks called me to say that he had received the significant deficiencies letter and that all deficiencies had been corrected. He said he had spoken with Meredith Reiley, Clatsop County Environmental Health a few weeks ago and discussed that the issues had been resolved. I told him that we would need a photograph or similar documentation to demonstrate that it was corrected and that Meredith could provide further information since she had conducted the original survey. He indicated he would follow up with her and the site manager to make sure the actions were completed. | ||
