| Details: | I contacted the operator, via email, to let him know the deadline for correcting the deficiencies found during the survey was approaching. I provided a copy of the deficiency summary and my contact information if he had any questions. 06/12-contacted operator via email about the overdue deficiencies. I inquired whether any of the deficiencies had been corrected and to please let me know if they had. I also offered to assist with completing any of the requirements due. 09/17 Contacted operator via email requesting a call back. |
