| Details: | Triggered and repeat samples positive Coliform. A second Level 2 Investigation has been done. Discovering that from the first Level 2 investigation, one last correction was not made within 30 days. A Tier 2 Public Notice template has been provided to the facility that carries a Traveler's Accommodation License with OHA. Spoke with Tracy whom has contacted a plumber to install UV rated NSF 55a for residual maintenance. Highly recommend going through plan review with the Oregon Health Authority, contact provided. | ||
