| Details: | I contacted Julian Ochoa about his assigned Level 1 investigation. He notified me he had mailed it to ODA main office in Salem. I contacted Salem and they scanned and forwarded me the completed investigation. Level 1 Investigation from COLI 17754 was received 04/23/2018. Multiple potential sources of contamination were identified by the operator. I discussed these sources and their possible solutions with the operator. |
