| Details: | I emailed the operator the 120-day letter, a Tier 2 Public Notice, and their 2025 survey outlining the overdue significant deficiencies. The previous corrective action plan that the operator agreed on was 8/8/25 and I've yet to receive any proof of deficiency corrections. When I've called the operator's phone and the facility phone they will ring, go to voicemail, and the voice mailbox is full. 9/5/25: I physically mailed the survey, survey letter, 120 day letter, and public notice to the operator using Certified Mail through USPS. + 9/6/25: USPS tracking shows that a notice was left for the recipient and no 'Authorized Recipient Available'. ++ 9/11/25: USPS tracking shows 'Reminder to Schedule Redelivery of your Item'. ++ 9/19/25: USPS tracking shows 'Return to Sender' +++ 11/4/25: I've received no response from the operator. |
