| Details: | 12.10.2025 - The operator emailed me requesting a six-month extension to create their operation and maintenance. The request for a time extension is due to recovery from a long furlough and holiday staffing constraints. Next steps: DWS has granted a time extension for the system to complete an operation and maintenance manual. The new due date is July 31, 2026. All other compliance requirements will be met on time (January 6, 2025). 09.02.2025 2025 Survey site visit completed on 08/14/2025. Operator and DMCE notified on 09/02/2025. Unmet rule requirements and deficiencies due Jan 6, 2026. No changes to inventory needed. Survey RPT and LTR saved on S Drive. |
