| Details: | The following email was sent to Mandip Kaur. A positive coliform result was submitted for the routine water sample of 8/21/25. Coliform results available online at https://yourwater.oregon.gov/dcoliform.php?pwsno=95499 indicate the required follow up testing was not completed. The store is required to submit 3 repeat sample results and 1 triggered sample result. The store has missed the deadline for submitting the follow up test results and is now required to complete and submit a Level 1 investigation. The form has been mailed to the store. Please complete the Level 1 investigation form and email to me prior to 10/5/25. If the store did complete the required 3 repeat samples and 1 triggered sample, please email the results to me.
