Details:	Spoke with operator regarding the new test results. Site visit scheduled for September 12, 2025 for a Level 2 investigation and water system survey. September 12, 2025 site visit did not reveal cause, issues possibly with ATT System. A repair was completed on or around the same day as the samples were taken. Repair is approximately 45 feet up hill from the well head. E. coli in the distribution system but total coliform at the untreated tap at the well head. Request for on-site septic staff to come to the site. Met with Melvin (operator) and Dean (property maintenance personnel). Level 2 submitted. September 15, 2025. Met with county on-site septic staff. Potential issues with ATT system. This is a DEQ system and this entity was notified. On hold to determine if there are any issues with septic treatment. Melvin also is working on figuring out if there is an issue with any of the buried lines. October 1, 2025: Spoke with Melvin and he was able to get to Water Resources and has found out the well log was listed in Yamhill County and not Washington County. Well was drilled to 140-feet with a 18-foot seal. Basalt at 50-feet. They will not be replacing the lines since he does not feel this was the issue at this time. There was no loss of pressure. October 10, 2025: Spoke with Melvin and he stated they are not going to work on the lines. He submitted samples from the distribution system. October 16, 2025: Spoke with Melvin since the samples have not been uploaded online. He stated they came back absent but he didn't label them as routine. Let him know he can contact the lab to label them as routine or take additional samples. The facility is still on a boil water notice. Melvin will be on vacation soon, so it was asked of him to take care of the sampling or re-labeling before leaving.
