| Details: | Firm has multiple overdue deficiencies from the most recent survey conducted on 5/7/25. These deficiencies were due to be corrected by 9/24/25. Attempts at contacting the owner were made via voicemail on 9/23 and 10/6 and by email on 9/23. The owner contacted me via phone on 10/8 and we are working to schedule a visit where he and the property owner can meet me on site to address deficiencies. | ||
