| Details: | Emailed the system's listed contractor, notifying them that a level 1 investigation is required. The last round of repeat Coliform sampling was positive for Total Coliform. A Level 1 investigation must be submitted to Lane DW within 30 days. This system also neglected to submit a triggered sample within the required time limit after the last routine coliform positive, and should do that as soon as possible. The system should also provide current contact information including name, phone number, mailing address, and email address so that Lane DW can reach out as necessary. |
