Details: The plan review records have been updated in SDWIS and now show an approval date of 08/31/2016. The significant deficiency for no plan review approval for SRC-AA, Well-L123395 noted during the WSS was noted as corrected by me on 04/23/2019. The facility name will be changed to Well-L123395 from Well (Unapproved) through the submitting of an inventory change form.
