| Details: | Cross Connection Report summary and Screen on Tank No.2 On 04.12.19 I emailed Ms. Morris reminding her to mail their CCR certification statement to OHA-DWS. Ms. Morris informed me that their CCR certification report was mailed to the DWS on March 30, 2019. Regarding the missing screen on the storage tank No.2, Ms. Morris stated that she still needs to send me a picture of the replacement screen on the holding tank drain. On 05.06.19 I emailed Ms. Morris reminding her to email me a picture showing the screen replacement installed on the storage Tank No.2. On 05.10.19 Ms. Morris emailed me a picture of the screen that was installed on the holding tank drain pipe. The operator - Mr. McDonald, confirmed over the phone that the screen was installed on 04.01.2019. |
