| Details: | Called John and left a voice message regarding the repeat sample that tested positive for TC. Informed him that this will trigger a Level 1 Investigation. I also mentioned that 2 other repeat samples and a source sample still need to be taken/reported, as well as 3 temporary routines before the end of February. This information was also repeated in an email sent to John and Micah shortly after. A copy of the L1 Form was attached. |
