| Details: | Having been assigned to Polk County in October 2024, I conducted a water system survey on December 19, 2024 in which I discussed unresolved violations with Stan Mills, I sent an email on April 25, 2025, with a copy of the survey report and pointed out violations for not submitting the calendar year 2024 Consumer Confidence Report (CCR) as well as for not submitting the annual cross connection control Annual Summary Report (ASR) for 2022, 2023, and 2024. I referred Mel to Civil West (SRF technical assistance circuit rider) for assistance with resolving these violations. Mel Wheeler passed away on December 2, 2025, so I sent updated contact information to Dan Vaage with Civil West on December 18, 2025. I also sent a list of items Rock Creek needed assistance with to Emelia Watts with Civil West on January 21, 2026, which included completing the CCR and ASR reports. I sent Stan Miller (board president) some example CCRs and the CCR checklist on February 12, 2026. After working with Stan Miller, the board president and Civil West (Dan Vaage and Emelia Watts) The ASR for calendar year 2025 was ultimately received on February 23, 2026. | ||
