| Details: | System violated TCR 6/2018 with not enough repeat samples. System did not submit Level 1 investigation for and did not issue PN for the violation. Butch was in office 5/23/19 and filled out Level 1 and developed PN for the event. I will send documents into Compliance. Also discussed missing DRC, CCR and ASR. Butch will have owner Dave Hancock sign form and CCR and ASR were sent out but not to state and no verification form. | ||
