Details:	I performed an onsite inspection of the ODFW facility in Leaburg and all items discussed during the previous level 2 investigation have been corrected. The system has since installed a UV light treatment system with iron removal to assist in the coliform issue at the facility. This has gone through plan review. The system can return regular quarterly sampling.
