On 06.28.19 I contacted Ms. Helen Bowman and informed her that the water sample collected on 06.25.19 came back positive for TC. I daviced Ms. Bowman to collect 3 repeat samples and 1 triggered sample as soon as possible. Samples should be collected at the following locations:

a) One at the same site that tested positive (Lodge).
b) One within five (5) connections upstream from the site that tested positive.
c) One within five (5) connections downstream from the site that tested positive.
d) One at the wellhead (raw water that hasn't been treated). Label this sample TG (Triggered) .

Operator was also advised not to pour chlorine into the well or attempted any kind of treatment to correct the issue prior to collecting the samples.

On 07.08.19 I contacted Mr. Toby Bowman to find out if the TG sample and repeat samples were collected and taken to the lab prior to July 6, 2019. Mr. Bowman stated that the water samples were collected and sent to the lab on July 1st, 2019. All samples came back absent. I also contacted the lab and they informed me that the sample report will be sent to the DWS today. A Level 1 investigation is not necessary. I told Mr. Bowman to dismiss the letter from the state requesting the completion of a L1 investigation.
