Details:	Richard completed the Level 1 investigation form and delivered it to the office. I reviewed it and it was determined the first total positive was the result of sampling errors, including unnecessary exposure of sampling vial and inconsistent sampling procedure. The repeat positive was the result of sampling from a swivel faucet and failure to adequately flush prior to sampling.
