Details:	Today contact was attempted to be made with the operator at the facility, by cell phone, and by email regarding survey deficiencies. A message was left again with the staff at Rogue Valley Stations and an email was sent to the operators mom. The email contained the previous survey information including deficiencies and how to correct them.
