| Details: | The system had a positive routine sample on 8/23/19. Tim took the required follow-up samples but they were submitted late. I received a notice from Chuck Michaels that because the samples were late the system would be required to conduct a level one investigation. I explained to Tim the template would be in the mail with a compliance letter and it would need to be completed and submitted within 30 days. I offered to assist him with the investigation template if needed. | ||
