Details:	Today phone contact was made with the operator regarding a unaddressed significant deficiency from 2017. The operator had apparently switched usage of wells on the property to his residential well, and never went through the required plan review process for the major modification. It was discussed that the operator is still required to undergo plan review with a plan review engineer associated with the Oregon Drinking Water Program. Emailed plan review materials to the operator and made contact with the plan review engineer via email.
