Details:	Today phone contact was made with the operator's mom who is the owner of the firm. It was explained that her son (the operator) has neglected to return several phone calls regarding unaddressed significant deficiencies. It was explained that the facility has two significant deficiencies that need to be addressed including the well setback from the well being located too close to the fuel pump island. The facility also has their wellhead in a sunken vault that does not drain to daylight. It was explained to the owner that 30 days have been given to address deficiencies or progressive action may be taken. The owner explained that she should be listed as the operator and system contact. A system info change form was submitted to compliance. The owner/operator requested that the October 3rd, 2018 survey be mailed again to the firm. The survey has been mailed again today from the ODA Salem office. In addition, an email was sent to the owner/operator regarding the unaddressed deficiencies and options for correcting these deficiencies.
