Details:	Today phone contact was made with the operator as the facility has now missed their monthly coliform monitoring for October and November. It was discussed that this facility is responsible for monthly coliform monitoring of their distribution. It was also discussed that we are nearly half way through December and a sample has yet to be submitted. Operator understands that a monthly sample must be submitted for this facility.
