Details:	Today I attempted to contact the operator of the facility by contacting the facility's phone number. Again, I left a message with the staff. I also sent another email to the operator regarding the requirement to address the sanitary seal that is not water tight. In addition, I discussed again that the facility did not submit 3rd quarter coliform test results from the distribution and have yet to submit coliform results for the 4th quarter. It was explained that if the 4th quarter coliform test is missed, the firm will be put on monthly coliform testing.
