| Details: | SUMMARY: Monthly test positive, 4 RP all positive and two positive FC. DETAILS: We received the alert from DHS- DWP on 2/7/06. I called the operator on 2/7/06 and he said that he had disinfected the system and done four more repeat tests on 1/24/06, all four absent for coliform. I I told him he needed to do a Tier 2 public notice and gave him a copy of the template, asked him to fill it out and give me a copy of it. I would send it to DHS-DWP. I reminded him to do five repeats in February. ACTION NEEDED: I discussed with the operator that if any of the February routines come back positive for coliform, they will be a SNC.
