| Details: | I received notification from Sarah Schwab (ODA) that a Drinking Water Services mailer was returned to the program. I contacted the local Food Safety Specialist with the ODA, Anna Hyde, to confirm the Retail Food Establishment had closed, as it had been closed in the ODA license program. Anna contacted me via email to let me know that this was indeed the case and a Water System Information page was completed and sent to compliance.dw@dhsoha.state.or.us to deactivate the water system in SDWIS by me on 01/28/2020. | ||
