| Details: | SUMMARY: ERP Final Review DETAILS: I met with Alvin at McLain Village on 4/6/06 to review his competed ERP. The plan appears to meet the minimum requirements. I suggested he add Marion County EH to his contact list. I instructed Alvin to complete the certification form and send it to Kurt Putnam at dHS DWP. I gave him a pre-addressed envelope to do so. ACTION NEEDED: Revisit ERP during next sanitary survey. | ||
