| Details: | SUMMARY: System has not submitted a CCR DETAILS: This system was referred to the Circuit Rider program for failure to prepare and submit CCR. ACTIONS PERFORMED: Called Zelenka to discuss the need for the CCR ad to make an appointment for a field visit. Pulled pertinent water quality data for this system off the state data base. Visited Zelenka and showed him how to access the state DWP website. Pulled up the CCR writer website and logged on. Created a CCR for 2005 using CCR Writer and the pertinent water quality data. Called up and printed the sample certification form. I explained to Zelenka that this is an annual requirement and must be done by July 1st of each year. ACTION NEEDED: Mr Zelenka plans to distribute the CCRs to residents during December. He will either mail them out with the next billing or hand deliver them. After they are mailed out, he needs to submit a copy to DWP along with a completed certification form. | ||
