| Details: | SUMMARY: Operator had 4 repeat samples taken but only one tracking slip was filled out. Lab could not run the other 3 samples. DETAILS: Routine TC test on 10/12/06 positive. 4 repeats were pulled but 3 lab slips were missing. The 1 repeat was positive so I asked the operator to post the system. ACTION NEEDED: Posting was removed after the 3 repeat samples came back negative 10/23/06. 5 routine samples are needed for November. | ||
