| Details: | SUMMARY: TC pos. alert attributed to data entry error. DETAILS: Hood River County Health Department received an alert by fax on 10/23/06 for Meadows Utilities LLC in response to TC pos. sample result (ID: PPJ0471-06A, collected 10/11/06). Patricio Ramos was contacted by email and phone on 10/24 and 10/31. The positive result is attributed to a data entry error at the contract laboratory. On 10/25/06 Meadows Utilities LLC CSI discovered that the sample collected 10/11/06 for PWS no. OR4191163 was logged into the labs LIMS system (Element) correctly, but had been entered into the state drinking water form incorrectly. The correct sample number associated with PWS no. OR4191163 is PPJ0471-07A instead of PPJ0471-06A. The sample ID PPJ0471-07A test result was absent for coliform. ACTION NEEDED: Industrial services manager Brian Cone, CHMM at Test America corrected the state form to document the correct sample ID. | ||
