I contacted the city and found out that Bob Lewis was the new contact and he did not have any 2007 form in hand as he recently replaced Jim Wear who would have received it in March. I contacted George Waun at DWP to email copies of the survey and instructions in order to complete. ACTION NEEDED: None | ||
