| Details: | SUMMARY: State Reg: Routine Taken 10/23/07 +TC DETAILS: 10/29/07: Called number on alert for Hugh Hill-- no answer and no message machine. Tried searching White Pages, no luck.11/1/07: Called Mark Leeds at Coffey Labs aand asked for contact info for this system. He gave me name and # of Doug Rosario at 503-688-9849. I called that number and left a message stating that four repeats should be taken asap and then 5 temp routines required in November. I also mentioned that if any repeats are positive than a PN is required. I left my name and number and asked Doug to call if he has any questions. ACTION NEEDED: No further action needed at this time. | ||
