| Details: | SUMMARY: Operator Will Post PN, M/R Five Routines in Nov. 07 DETAILS: I contacted the operator re the positive TC repeat sample collected 10/22/07. The operator states he will provide PN as required -- I faxed him the forms. We discussed possible causes of routine and repeat at same sample site -- plumbing work done without disinfection, leak in area, cross connection.
