Details:	SUMMARY: TCR not reported for 2nd and 3rd quarter 2007, 4th quarter 2006, and 4th quarter 2005 DETAILS: 12/21/07 Contacted the operator and discussed missing records. Gary ( another operator) returned the call and promised to hand-deliver copies of reports that same day. We also discussed the need to report results on time for the following 2 quarters to avoid additional violations. Gary supplied copies of the 2nd and 3rd quarter samples from 2007 and also reported the 4th quarter samples for 2007 on time to the State. He had the Chain of Custody paper and receipt from the 2006 and 2005 missing samples, but did not have copies of the results. We also discussed the need for communication between him and the lab to submit the sample results to the State. ACTION NEEDED: Achieve compliance by 12/31/08 by reporting the quarterly results on time.
