| Details: | SUMMARY: ERP on site consultation DETAILS: During prep for sanitary survey I noted there never was an EAP/VA completed for the system. I took the forms for both and reviewed them with Mr. Beauchamp. We discussed what portions applied and went over the instructions on completing the EAP/VA. Set date for completion of plans by 5/30/08. ACTION NEEDED: Follow up on progress and completion by 5/30/08. | ||
