| Details: | SUMMARY: none DETAILS: During the survey on 2/20/08 Mr. Pruner said he had completed the VA. He did not have a copy and said he sent it to the state. Douglas County has nothing in his file to indicate it was completed. I went over how to complete the VA at the time of the SS visit. I called DWP to see if per chance the VA was there. No luck. I did however find out that the water systems must complete both the VA and the ERP. I called Mr. Pruner to let him know he needed to do more than the VA. I took the ERP forms out to him on 4/16/08 and explained the purpose and that they were to be useful for him. He completed both and I reviewed them . He has copies in his water system file. Also confirmed all items needed to be completed on SS by March 15 were done. The coliform sampling plan, the map of distribution system, nitrate sample results for 2006,02007, and 2008 were provided to me. We discovered that Umpqua Labs does not send nitrate results. That was something I did not know. I gave the Pruners the DWP contact info so that they can provide the missing test results to you. ACTION NEEDED: none | ||
