| Details: | SUMMARY: Violations incurred for failure to submit routine coliform samples. DETAILS: 7/21/08 I called the church and was given Matt Sapps (pastor) home phone number - 541-826-9996. I left a voice mail requesting that he call me back. 7/22/08 I spoke with Matt about sampling protocol. He submitted a routine coliform sample and an annual nitrate sample. 7/24/08 The routine coliform sample tested negative. ACTION NEEDED: Routine coliform samples need to be collected on a quarterly basis. | ||
