| Details: | SUMMARY: Failed to report IOC testing DETAILS: April 2, 2008 I left a voice mail for Jeff to contact me. April 4, 2008 I spoke with Jeff & informed him of the violation for IOC testing. He will contact me when the lab completes the tests. June 27, 2008 I spoke with Jannett, Jeffs assistant. She had contacted the lab & had a question from the lab about what IOCs needed to be tested. I read off the required IOCs from SIDWIS. Sept. 15, 2008 I contacted Jeff to inform him that the test results are not on SDWIS. He said the lab told him they would send them in.
