| Details: | SUMMARY: Failure to submit annual nitrate sample in 2007 DETAILS: 8-22-08 I left a voice mail for Stewart explaining the violation. 9-8-08 - I verified with Neilson Research that a sample has not been submitted. I left a voice mail for Stewart reminding him that a sample needs to be submitted as soon as possible. 9-16-08- I verified with Neilson that a sample has not been submitted. I left a voice mail at the church 541-826-3837. 9-22-08 - I verified with Neilson Research that a nitrate sample was submitted on 9-18-08. ACTION NEEDED: No action is needed to return this system to compliance. | ||
