| Details: | SUMMARY: Failure to submit an IOC sample set in 2005-2007 compliance period. DETAILS: 8-19-08 I left a voicemail for Tom explaining the violation. 9-8-08 I verified with Neilson research that a sample has not been submitted. I left voice mail for Tom reminding him that a sample needs to be submitted as soon as possible. 9-12-08 I spoke with on site manager Carrie Youtsey (541)878-2463. She informed me that Neilson Research collects all of their chemical samples. 9-15-08 I called Neilson and scheduled an IOC sample collection. 9-18-08 Neilson Research collected an IOC sample. ACTION NEEDED: No action is needed to return this system to compliance. | ||
