| Details: | SUMMARY: Violation DETAILS: System violated 4th quarter 2008 non reporting. Called and number is disconnected. Called lab and no sample for 4th quarter. Another staff member finally contacted system regarding restaurant license and facility still in business. System took 1st quarter 2009 sample 1/13/09. Results not posted until March due to some billing issues with lab. ACTION NEEDED: Continue to take routine quarterly coliform and annual nitrate. | ||
