Details:	SUMMARY: Coliform reporting DETAILS: I have called twice and left messages for Joe in regards to lack of reporting results. I visited the firm and left information on the reporting and to call me on the results. I encountered the sample form and provided numbers for the sampling to be sent. This is the second quarter that results have not been reported. ACTION NEEDED: Operator needs to follow up with corporate personnel to ensure that the proper numbers are being used, reports are being made and test results are being sent correctly to the state.
